Continuing Education Tracking Program

END USER LICENSE AGREEMENT:

Please read the following terms and conditions carefully before using this Tracking Service.  Your registration and use of this product indicates your acceptance of this agreement.

1.  OWNER: This product is owned by the National Association of Social Workers-Michigan Chapter.

1.1 | RIGHTS OF OWNERSHIP: NASW-Michigan and its suppliers reserve all rights not expressly granted to you in this EULA.  This product is protected by copyright and other intellectual property laws and treaties.  NASW or its suppliers own the title, copyright and other intellectual property rights in this software.

1.2 | LIMITATIONS ON REVERSE ENGINEERING, DECOMPILATION, AND DISASSEMBLY.  You may not reverse engineer, decompile, or disassemble this program, except and only to the extent that such activity is expressly permitted by applicable law notwithstanding this limitation.

1.3 | ADDITIONAL SOFTWARE/SERVICES:  This EULA applies to updates, supplements, add-on components, or Internet-based service components of this program.

2.  USER:  This product is for use by Licensed Michigan Social Workers for tracking continuing education hours, intent for purposes of licensure renewal in the State of Michigan.

2.1 |  INTERNET BASED SERVICES:  You may not use any internet-based service in any manner that could damage, disable, overburden, or impair such services or interfere with any other party’s use and enjoyment of these services.  You may not attempt to gain unauthorized access to any service, account, computer system or networks associated with the internet-based service.

2.2 | INTERNET SERVICE PROVIDER:  User will supply and use their own Internet Service Provider.  Fees for the tracking service do not include internet access, DSL, cable, modem, or dial up services etc…

2.3 | ONLINE SERVICES:  Certain functionality of this tracking program may require separately acquired on-line services.  Neither this program nor this EULA gives the USER any rights to use the Internet, any on-line or other services or software that may be necessary to use all features associated with this program.

2.4 | CONSENT TO USE OF DATA:  You agree that NASW-Michigan may collect and use technical information gathered as part of the product support services provided to you, if any, related to this program.  NASW-Michigan and their affiliates may use this information solely to improve their products or to provide customized services or technologies to you and will not disclose this information in a form that personally identifies you.

2.5 | LINKS TO THIRD PARTY SITES:  NASW-Michigan is not responsible for the contents of any third-party sites or services, any links contained in third-party sites or any changes or updates to third-party sites or services.

2.6 | HARDWARE COMPONENTS:  NASW-Michigan nor its affiliates are responsible for the performance or functioning of any of the USER’S computer or hardware systems.  Technical support will only be offered for functions associated directly with the operation of the software tracking program.

2.7 | USER agrees not to sell, resell, license, rent, lease, lend or otherwise transfer for value any part of this tracking program.

3.  FEES AND PAYMENTS:

3.1 | Members: NASW-Michigan Chapter members are eligible to use this product for a one year period at a cost of $0.00.  One year from the date of registration a fee of $35.00 will be assessed for those members who wish to maintain the continuing education tracking services.

3.2 | Non-Members:  Social workers who are not members of NASW-Michigan may contract to use this continuing education tracking service at the cost of $45.00 per year.  First year begins with the date of your registration and acceptance of this END USER AGREEMENT.

3.3 | Non-Payment:  NASW-Michigan owns the database files that contain the continuing education records and reserves the right to deny USER access to those files for non payment of services.

4.  SERVICE DESCRIPTIONS:

4.1 | Tracking guidelines are based on the State of Michigan requirements of 45 continuing education contact hours for a three year licensure cycle.

4.2 | All social workers who have registered to use this product understand that they are solely responsible for registration, payment, attendance and completion of all course requirements.

4.3 | NASW-Michigan or any affiliates will not be held responsible for any user’s neglect in meeting the continuing education requirements.  Including but not limited to, reporting required CE Clock Hours to the State of Michigan.

4.4 | NASW-Michigan reserves the right to cancel USER services for misuse of service; or fraudulent use of service.

4.5 | TERMINATION:  Without prejudice to any other rights, NASW-Michigan may terminate this EULA if you fail to comply with the terms and conditions of this EULA.
